The Staff Development team spent months planning a needed overhaul of the routine home health visit documentation.
To effect a successful go-live, challenges included sufficient communication, equipping staff with the tools to deeply understand the new system and incorporating feedback and improvements in real time.
My contributions to these change management efforts included:
-Creating a unique, unifying theme for the project that stood out starkly from routine communications.
-Standing up a dedicated resource center in the Sharepoint environment featuring collected reference materials and updates.
-Planning and executing a communications strategy to include justifications, reminders and signposts to supportive resources.
-Innovating a detailed, interactive, self-guided reference document for staff to explore the complex pathways.
